Job Description:

Responsibilities / Activities: The independent assessment, treatment and discharge planning of patients in community rehab team, Equipment reasoning and ordering, use of recognisable outcome measures, family meetings.
Skills / Experience: Experience in NHS and post qualification as an Occupational Therapist, preferably within the community and rehab setting. Car drivers would be required given the community role.
Monday - Friday, 08:30-16:30, 37.5 hours per week. Would consider part time working minimum of 3 days (22.5 hours) dependent on experience.
